Practice Reviews
There is an obligation for all agencies involved in the care, support and protection of children, young people and adults at risk to ensure that the highest possible standards of care, support and protection are provided and maintained at all times. Part of this obligation is a requirement to learn from mistakes, especially those resulting in death or serious injury.
In accordance with the Social Services and Well Being (Wales) Act 2014, the Cwm Taf Safeguarding Board have a responsibility to establish a Practice Review when the criteria requiring one are met.
A Practice Review is held if;
- abuse or neglect is known or suspected and the individual has died;
- or sustained potentially life threatening injury;
- or sustained serious and permanent impairment of health or development
Domestic Homicide Reviews
A Domestic Homicide Review (DHR) is a multi-agency review of the circumstances in which the death of a person aged 16 or over has, or appears to have, resulted from violence, abuse or neglect by a person to whom they were related or with whom they were, or had been, in an intimate personal relationship, or a member of the same household as themselves.
Professionals who wish to refer a case for consideration of a Practice Review are asked to complete the appropriate Practice Review Referral Form below and return to the Safeguarding Board Business Unit via email ctmsafeguarding@rctcbc.gov.uk
